The participant voluntarily agrees to provide DNAisyou with a DNA sample, which comes from the participant him or herself, for the purpose of carrying out a DNA analysis by Omnigen BV and its licensee DNAisyou.
Omnigen BV will draw up a report for its licensee DNAisyou on the basis of the DNA analysis carried out, which will be made available to the participant. Although Omnigen BV and its licensee DNAisyou will exercise the greatest possible care in both carrying out the DNA analysis and drawing up the report, no guarantee can be given by Omnigen BV and its licensee DNAisyou with respect to the (accuracy of the) results of the DNA analysis, as well as the report. No rights may therefore be derived from the (accuracy of the) report by the participant.
Omnigen BV and its licensee DNAisyou is not liable for any damage that is caused by carrying out the DNA analysis and/or the report drawn up on this basis. In the event that an appeal for exemption from liability is judged to be unacceptable according to the standards of reasonableness and fairness, Omnigen BV and its licensee DNAisyou is only liable for direct damage that is the result of imputable acts on the part of Omnigen BV and its licensee DNAisyou. The total liability of Omnigen BV and its licensee DNAisyou is limited to no more than the amount that would be paid in such a case under the liability insurance taken out by it.
The participant gives Omnigen BV and its licensee DNAisyou permission to process the personal data provided on this form, as well as the data that is obtained on the basis of the DNA analysis, in order to be able to carry out the DNA analysis and provide feedback on the report.
Omnigen BV and its licensee DNAisyou respect the privacy of the participant and will handle this data confidentially and ensure adequate security of this data. Omnigen BV and its licensee DNAisyou will not pass on or disclose this data to third parties without explicit permission in advance from the participant.
Use of personal data.
Privacy sensitive data or personal data is processed via the website of DNAisyou. DNAisyou considers the careful handling of personal data to be of great importance. Personal data will therefore be carefully processed and protected by us.
During our processing, we will comply with the requirements that the Personal Data Protection Act sets. That means, among other things, that we:
